I have been to Hazel Dell family care two times and saw nurse practitioner Emma Vazquez. I cannot speak highly enough of her and how well she calmed me on both visits catching that I was very sick the first visit and sent me straight to the hospital. I just visited to have a check up/well care visit and she took the time to listen and did not push for me to have anything more than blood test until I saw my oncologist again. To reiterate, I cannot speak highly enough of her. She made me feel very comfortable.

N.P. Velazquez always takes my concerns seriously. I have had a few times where other doctors wouldn’t listen to my concerns or issues and then I would go see her and she would listen and help figure out what’s going on. This time around I talked to 2 other doctors before seeing her because I wasn’t being taken seriously, N.P. Velazquez listened and took the proper steps to figure out what was going on. Now I’m on medications to fix my problem. I will always see her as long as I can since I know I can rely on her to help me!

Emma has a special way of making her patients feel seen, heard and understood. She takes her time and makes sure that I meet all of my follow up appt w specialist that she has referred me to. If I need to communicate through my chart, Emma or Tamara respond in a timely manner. I am treated with care, value and respect. Hands down, the best doctor Ive ever had. She advocates for me and makes sure that all my needs are met.

I was seen quick for severe ear pain and infection. Medication prescribed and advise given for prevention. Emma was friendly and professional.
This was my first visit and based on my first impression the lobby was clean and comfortable. The receptionist was friendly and polite. The medical assistant was professional and easy to talk to. The Dr was attentive, friendly, knowledgeable and professional. She listened to my concerns and she provided me with adequate feedback.
