I’ve been seeing Lori Jacobus NP since 2022 and I absolutely LOVE her! She is so sweet, makes me feel heard & not rushed, seems very knowledgeable about every topic I’ve brought up to her, and my favorite thing about her is that she always suggests a natural approach first before pushing medicines!I came to her a few years ago having what I thought were “stress headaches”. When I explained what was going on I assumed she would give me a medicine for it. But to my surprise she suggested getting a massage. I found a holistic massage therapist and afterwards my stress headaches went completely away!!!The facility always appears to be clean and I always get checked in quickly and get out in a timely manner! The nurses that have checked me in or gotten blood tests are hit or miss, some are friendly and some aren’t so friendly.10/10 would recommend!

Lori Jacobus has been a catalyst for significant improvement in my health. She’s wise, thorough, & encouraging. And she’s taken time to talk to me about & refer me to Riverview resources and services I didn’t know about. The office staff people are kind & helpful. The lady who checked me in/out yesterday was awesome- I should have gotten her name. She helped with a scheduling situation & also took the time to figure out why I couldn’t get into the new app & got it so I’m finally able to log in. They’re always responsive to messages, questions, etc. I’m so glad to have found this office & especially Lori. A lot of people avoid going to the doctor & bringing up their concerns, etc. when practitioners are blasé or dismissive, the office has a bad atmosphere or they feel rushed / like “just a number” not a person. I know my health has significantly benefited because of how Lori Jacobus has treated me & how connected and cared for I am at Hazel Dell family care.
Never have had to wait an unreasonable amount of time. Staff is friendly and helpful. Lori is always attentive and helpful to my needs

I switch back after going to Community because it’s closer to home. I didn’t receive the level of care I get with the doctors at Hazel Dell nor was I listened too like they do there. It made the choice easy to just make the longer drive to make sure I’m taken care of.
Everyone in the office, is very helpful and friendly.Lori Jacobus (NP) and Aireole (asst/floater) were so very attentive and listened to my ailments.I got an x-ray and the 2 ladies that did it were so professional and concerned about my pain.
